All students participating must have a current Medical and Liability Release form on file at the church. If you (or your guests) do not have one on file, please click the link below to download the form, then fill it out and return it to Daren. You may do that by bringing it to the next youth gathering, or by scanning it and emailing it to daren@startlifeover.org.

CCC Medical and Liability Release Form

The cost for this event is $20 per student. You may pay that by dropping a check in the offering on Sunday morning or handing a check to Daren (please note CRave Fun Day in the memo), or you can pay online. Please note that there is a $1 per student fee added if you pay online to cover the fee charged to the church for the online transaction.
